We utilize a HIPAA compliant Electronic Health Records platform. New client forms will be sent to you via our online portal when you schedule an appointment. Forms will also be available to  complete at our office if you are unable to complete them online. If filling out forms in-person, please arrive . All forms are required at the time of your first appointment.
